Professional or employment information, if still employed, such as job title, employer, business address and contact information.

In recorded communications with MedicareWizard, personally permitted by you, and at your request, MedicareWizard’s licensed agents must share your information with an insurance carrier in submitting your application for Medicare or Supplemental coverage. In some instances, with your permission provided, our staff or agents may need to share your information to verify prescription drug coverage, pharmacy or primary care information, to help you determine plan selection or prioritize Medicare coverage considerations or understand changes to current plan coverage.
Organizations with which you have a relationship: In instances of clarifying plan coverage, or helping you with claims issues or related plan coverage circumstances, you may engage us to work with you and the third party to clarify a matter or resolve an issue. In these instances, with your permission we may share personal information, as needed, with the party - affinity group, employer, provider group, benefits administrator or consultant, healthcare system, pharmacy, or other entity to help you resolve the matter.
